Student-Athlete Insurance Information

All student athletes at Virginia Tech are required to show proof of insurance. If you do not have health insurance please contact your Athletic Trainer for options. There is information on this form that will require participation of your parents. Please proceed to complete your insurance information. You must complete the process for your insurance information to be accepted. You can leave this form at anytime and resume where you left off.

You must have a Virginia Tech PID and be enrolled in 2-Factor Authentication to use this site.
